What can you report here
A person bound by a site management order (SMO) can use this application form where they believe that changes in circumstances or conditions at their premises mean that amendment or revocation of the order under section 277 of the Environment Protection Act 2017 is appropriate.
Please note when submitting this form that an application fee of 103.29 fee units applies (approximately $1,550), and that if assessment time exceeds more than 14.9 hours then an assessment fee of 6.94 fee units will be charged for each additional hour (or part thereof).
SMOs are ineligible for extension of time requests. If you are unable to meet any requirements, please call 1300 372 842 (1300 EPA VIC) to discuss this with an authorised officer from the relevant EPA business unit.
